Northwest Health Services is committed to serving the unmet needs of individuals in your community. By developing partnerships with other agencies and organizations in the area we are able to offer specialized services to make services more accessible to everyone, regardless of their ability to pay. Some of these special programs include:

Outreach

Northwest Health Services is an active participant in all the communities we serve. By working with other community agencies and organizations, Northwest Health makes it easier for you to access the services you need. Medical care, nutritional counseling, dental care, and health education brought to everyone in the community regardless of language or ability to pay. Diabetic Education

Northwest Health Services offers free diabetic education classes and personal consultations with our Certified Diabetic Educator, Tara Stevenson RN, BSN, CDE. Each class covers a different topic essential to diabetes self-management. HOME Unit

Everyone in your community deserves quality attention. The Northwest Health Services HOME Unit coordinates preventive and primary medical, dental, mental health and substance abuse services, eligibility assistance, and housing assistance for homeless persons. The team includes a Family Nurse Practitioner, a Registered Nurse, a Dentist, a Dental Hygienist, a licensed clinical social worker, as well as an outreach worker and two case managers.
