By developing partnerships with other agencies and organizations in the area we are able to offer specialized services to make services more accessible to everyone, regardless of their ability to pay. Some of these special programs include:

Outreach

Northwest Health Services is an active participant in all the communities we serve. By working with other community agencies and organizations, Northwest Health makes it easier for you to access the services you need. Medical care, nutritional counseling, dental care, and health education brought to everyone in the community regardless of language or ability to pay. Nutrition Education

Northwest Health Services has a team of professionals ready to help you plan a healthy diet, or create a lifelong nutritional plan for your special needs. Schedule a private consultation or attend one of these convenient sessions near you.

Chronic Care Education

Living with your medical diagnose successfully is easier when you know what to expect. Northwest Health Services works with you to make sure you have the tools and skills you need to manage your diabetes, heart disease, pain and other chronic health conditions. Personal consultation is available. You can also attend one of these helpful workshops or support groups.

HOME Unit

Everyone in your community deserves quality attention. The Northwest Health Services HOME Unit coordinates preventive and primary medical, dental, mental health and substance abuse services, eligibility assistance, and housing assistance for homeless persons. The team includes a Family Nurse Practitioner, a Registered Nurse, a Dentist, a Dental Hygienist, a licensed clinical social worker, as well as an outreach worker and two case managers.
