Making an Appointment

Locate the Northwest Health Services clinic nearest you. Then call to schedule an appointment or contact us via email. Be sure to include your phone number on all communication so that we may contact you to confirm your appointment. All Northwest Health Services Clinics accept patients regardless of ability to pay.

Northwest Health Services accepts Medicaid, Medicare, most private insurance, and offers a discount program for those who are uninsured and underinsured. The discount program is based on household size and income. If you’re interested in applying for the discount program, please complete the Discount Program Packet 2012 and bring it with you to your first appointment. You can also complete your New Patient Packet prior to your visit.

Except in urgent situations, we would like you to call several days ahead for an appointment. Preventive care, such as family planning, well-child exams and physicals, are usually scheduled two to three weeks in advance. If you need an appointment on the same day, please call early in the morning. We will do the best we can to see you.

To help us give you the right kind of appointment, please tell the receptionist enough information about your medical concern.

NOTE: Since we do not provide child care services, if you are bringing children who do not have an appointment, please bring someone along to care for your children during your appointment. Our staff cannot take this responsibility.

- When the Clinic is Closed

If you have an urgent health problem after regular clinic hours, call your clinic’s phone number. An operator will take your name, your phone number, the patient’s name, and the reason for your call. A nurse, doctor, or nurse practitioner will return your call as soon as possible to advise you about what steps to take at home, or will advise you to go to another clinic or hospital. Please call during regular clinic hours if your question or concern can wait.

- New Patients

On your first visit, plan to spend the first 15 minutes completing registration forms or complete the forms prior to your visit. They are provided here for your convenience. Print them out and bring them with you.

  • A list (or the bottles) of all your prescriptions.
