There may be many reasons someone does not have a Primary Care Provider (PCP) or a medical home. Whatever those reasons may be, Northwest Health Services understands the importance of people connecting with a Doctor or Nurse Practitioner and creating a relationship so that YOU (the patient) feels comfortable and at home.
Northwest Health Services is partnering with Heartland Health and Social Welfare Board (the free clinic) to provide the correct referral process in order to put patients where they would fit best. Part of this referral process includes connecting you with a care manager who will help you understand and navigate through the medical system.
